CONSENT TO TATTOO/PIERCING PROCEDURE - ADULT

I acknowledge by submitting this form that I am at least eighteen (18) years of age and that I have been given the full opportunity to ask any and all questions regarding the tattoo and/or piercing procedure. All of my questions have been answered to my full satisfaction, and I voluntarily consent to the procedure described below.

I specifically acknowledge and agree to the following:

• I am not pregnant or nursing.
• I am not under the influence of alcohol, drugs, or any substances that may impair my judgment or affect the procedure.
• I have disclosed to my tattoo artist and/or piercer any and all medical conditions, allergies, medications, or skin conditions that may affect the procedure or healing process, including but not limited to acne, scarring or keloids, eczema, psoriasis, moles, freckles, sunburn, rashes, infections, or immune system conditions.
• I acknowledge that it is not reasonably possible for the representatives or employees of this studio to determine whether I may have an allergic reaction to pigments, metals, gloves, soaps, disinfectants, or other materials used during the procedure.
• I understand that infection, scarring, or other complications are possible, particularly if I fail to follow proper aftercare instructions. I have received aftercare instructions and agree to follow them. Any touch-up or corrective work required due to my own negligence or failure to follow aftercare instructions will be at my own expense.
• I understand that variations in color, shading, line work, or appearance may occur between the design selected and the final result. I acknowledge that skin tone and skin type affect how pigments appear and heal.
• I understand that future skin treatments, including but not limited to laser treatments, hair removal, cosmetic procedures, or surgery, may alter or damage my tattoo or piercing.
• I acknowledge that tattoos and many piercings are permanent changes to my appearance. No guarantees or representations have been made regarding the ability to alter, cover, or remove a tattoo or piercing in the future.
• I affirm that I do not have any physical, mental, or medical condition that would impair my ability to consent or that would pose an unreasonable risk as a result of the procedure.
• I consent to the tattoo and/or piercing procedure and to any actions reasonably necessary to perform the procedure in a safe and professional manner.
• The use of numbing cream during a tattoo session is my choice and that as stated above that it is my resposibility to know how my body may react to my skins or any area applied.
