Consent Forms and Minor Information
Complete consent submissions are stored in a restricted WordPress submissions area. A minor’s name and date of birth are intentionally excluded from routine staff and customer confirmation emails. Marketing is directed only to the adult client or parent/legal guardian—not to a minor.
Email and SMS Choices
Email and SMS marketing are separate, optional choices. You may unsubscribe from email using the link in a marketing message. For SMS, reply STOP to opt out or HELP for help. Choosing not to receive marketing does not affect your ability to submit a consent form or purchase services.
